Hospice Care
We provide care to our patients wherever they call home, whether that is a private residence, nursing home, Assisted Living Facility, or other location. We understand that each patient and family’s journey and needs are different. As a Medicare-certified hospice, R and A Hospice is proud to offer all four levels of hospice care: Routine, Inpatient, Respite, and Continuous Care. We work with our patients and medical team to determine the level of care appropriate for each patient.
We offer a full range of home care services, including:
· Care coordination assistance from medical social workers
· End-of-life support and services from hospice specialists
· Hands-on medical care from specially trained nurses
· Pain and symptom control from palliative care experts
· Physical, occupational, and speech therapies
· Pharmacy services and medication management
· Nutritional support, including intravenous feeding and hydration
Who is a Candidate?
If you're well enough to be cared for at home, but you need additional assistance,
home health care may be right for you. It's helpful for people who are:
· Recovering from an illness, accident, or surgery
· Unable to perform daily living activities
· Undergoing treatment
· Disabled
· Managing a chronic disease
· Terminally ill.
